§ 12-9. Membership.

  • (a)

    Conditions of eligibility.

    (1)

    All general employees already covered under the city's retirement plan on the effective date of this article shall automatically continue participation under the plan without any further action being required.

    (2)

    For purposes of member contributions, the application for membership shall be considered to have been in effect from date of employment. For purposes of disability, however, an employee shall be considered a member and eligible for benefits hereunder only upon satisfactorily meeting any medical standards imposed by the board.

    (3)

    Withdrawal from plan. No member shall have the right to withdraw from the plan.

    (4)

    Change in designation of beneficiary.

    (a)

    A member may from time to time change his or her designated beneficiary by a written notice to the board upon forms provided by the board. Upon such change, the rights of all previously designated beneficiaries to receive any benefit under the plan shall cease.

    (b)

    A retired member may change the designation of a beneficiary up to two (2) times without the approval of the board upon forms provided by the board. Any additional change must be approved by the board. Upon a change, the rights of all previously designated beneficiaries to receive any benefit under the plan shall cease. The retired member must pay the full cost of determining the equivalent actuarial value of the benefit payable for any changes made pursuant to this paragraph.
